We understand that schedules change. If you need to cancel or reschedule your appointment, we ask that you provide at least 24 hours' notice.
Appointments canceled or rescheduled with less than 24 hours' notice will incur a $50 late cancellation/rescheduling fee. This fee must be paid before another appointment can be scheduled.

Yes.
Cosmetic Consultations: $250, collected at the time of scheduling.
New Self-Pay Medical Patients: $250, collected at the time of scheduling.
Established Self-Pay Medical Patients: $175, collected at the time of scheduling.
Cosmetic Procedures and Treatments: $100 deposit required when scheduling; applied toward your treatment.

Because we are a specialty dermatology practice and are not affiliated with a hospital system, payment is due at the time of every visit.
Patients are responsible for understanding their insurance coverage before their appointment. We encourage you to contact your insurance company to verify your benefits and determine your financial responsibility.
All patient balances are due at the time of service, including:
Copays
Deductibles
Coinsurance
Non-covered medical services
Cosmetic consultation fees
Cosmetic treatments and procedures
Copays are collected at check-in and are not invoiced.
If your deductible has not been met, we will collect the estimated patient responsibility for your visit until your deductible has been satisfied.

Yes.
We encourage every patient to contact their insurance company to understand:
Your deductible
Your copay
Your coinsurance
Whether your visit or treatment is covered under your plan
Understanding your benefits before your appointment helps prevent unexpected charges.

It is your responsibility to provide updated insurance information before your appointment.
Please notify our office and provide:
Your current insurance card(s)
Updated insurance information (including plan name, ID number, group number, and policyholder name)
Any referral or authorization required by your insurance plan
Failure to provide current insurance information may result in your appointment being rescheduled or you being responsible for payment as a self-pay patient.

In most cases, we are only able to address one primary concern per appointment.
This allows Dr. Dika to thoroughly evaluate your condition while complying with insurance guidelines for specialty medical visits.
If you have additional concerns, our staff will be happy to help schedule another appointment.

The following are examples of cosmetic concerns:
● Hyperpigmentation
● Uneven skin tone
● Fine lines and wrinkles
● Skin rejuvenation
● Cosmetic skin tag removal
● Questions regarding facial cleansing routines
● Skin care products and product recommendations
● Cosmetic skin care regimens
Cosmetic consultations are $250 and payment is due at the time of scheduling.
Insurance does not cover cosmetic consultations or cosmetic treatments. Payment is due in full at the time services are provided.
Treatment plans and cosmetic pricing cannot be provided without a cosmetic consultation.

Appointments for hyperpigmentation, uneven skin tone, and other cosmetic discoloration concerns are considered cosmetic services and are not covered by insurance.

General skin care recommendations, facial cleansing routines, skin care products, and recommendations based on your skin type are considered cosmetic consultations.
These visits are scheduled separately as a cosmetic consultation for $250.

Full Skin Check
A Full Skin Check is a comprehensive examination performed to evaluate your skin for:
● Skin cancer
● Precancerous lesions
● Suspicious growths
● Other medically concerning skin conditions
Mole Check
A Mole Check focuses on 1–3 specific moles that you are concerned about.
If you are interested in a complete skin cancer screening, please schedule a Full Skin Check.

No.
Cyst removal is generally not performed during your initial evaluation.
Dr. Dika will first examine the cyst to determine:
● Whether removal is medically appropriate
● The best treatment option
● Whether a separate procedure appointment is needed
If removal is recommended, a separate procedure appointment will be scheduled.

Most skin tag removals are considered cosmetic and are not covered by insurance.
Dr. Dika will determine if your skin tags qualify as medically necessary; however, the majority are considered cosmetic.

Most prescription medications require periodic follow-up appointments so Dr. Dika can monitor your condition and ensure your treatment remains safe and effective.
If you are due for a follow-up appointment, one must be scheduled before additional refills can be authorized.

If you would like your prescription filled at a different pharmacy, please contact your current pharmacy and request that they transfer your prescription to your preferred pharmacy.
The pharmacy can transfer most eligible prescriptions directly, making this the fastest and easiest way to update your pharmacy.
